My problem/suggestion is...

Order 25 - expense

In 25, the last box needs to either take the gender of the first box “at [his/her] expense” or be “at the [applicant/respondent]’s expense”, whichever is the opposite of the first box. So if the first box is the applicant and she is female the box would offer the choice between “at her expense” or “at the respondent’s expense”
